Scam or fraud: Bank of Baroda — Amount not debited from my account

Complaint Details:
I bought baroda health policy on year 2007, premium amount has been debited from my account annually, But on year 2015 premium amount not debited from my account and policy get lapsed.When i ask the same to National Insurance they said due to bank mistake amount not sent to us.
And when i ask the same to BOB Panki Branch Mr. Deependra shukla (Branch manager) replied me over phone:
“Might be Bank not get consent from customer due to which amount not debited and i am here (BOB panki branch) since last 2 months so i don’t have much information on this”
Now:
1) If bank get the consent from customer yearly so why not get the consent on all previous years. Except 2015, in all previous years amount has been debited from my account without my consent.
2) As per reply of Panki branch manager why he is not sure? why he is using the word ‘Might be’.
3) As per reply from OPERATIONS.KANPUR@bankofbaroda.com (15th Oct’16) they said the get consent from customer before debiting amount.
If this true, how the amount has been debited from my account since past years (2007 to 2014) as i do not give any consent to Bank.
